Services Power of Attorney

General Information

No appointment needed. Power of Attorneys can be sent by mail to be legalized, but the document should be legalized by another republic and the Secretary of State. To be mailed with the application form, Lebanese ID of both the applicant and recipient & a copy of the Drivers License of the applicant.

Applicants over the age of 70 must provide a medical report stating their mental competency

$50.00 USD for Special Power of Attorney

$100 USD for General Power of Attorney

  • Please fill out the above application form completely.
  • Please bring the following to your appointment:
    • ID of the applicant (Lebanese ID recommended).
    • ID of recipient (Lebanese ID recommended).
    • The applicant should come in person to the Embassy to sign the general power of attorney.
    • For general Power of Attorney a Bank Issued Cashier's Check or USPS Money Order Only in the amount of $100.00 per signature per applicant payable to the Consulate General of Lebanon.
  • Please fill out the above application form completely.
  • Please bring the following to your appointment:
    • ID of the applicant (Lebanese ID recommended).
    • ID of recipient (Lebanese ID recommended).
    • The applicant should come in person to the Embassy to sign the special power of attorney.
    • A Bank Issued Cashier's Check or USPS Money Order Only in the amount of $50.00 per signature per applicant payable to the Consulate General of Lebanon.
    • For Real Estate matters: a copy of the deed of property or a registry certificate (ifada ikaryah) must be presented.
    • For cars: A copy of the car registration record should be presented.
    • For bank accounts: The bank address and account number should be provided.
    • For divorces: A copy of marriage certificate or family civil record should be provided.
    • For Maids/Housekeepers: A copy of the passport of the housekeeper is required.
  • All Power of Attorney received by mail (Special and General) composed of more than one page MUST include the signature of the applicant and the seal and sign of Notary Public on each and every page of Power of Attorney.
  • All Power of Attorney should include the full identity of both applicant and recipient: First and Last name, Father's name, Mother's name, Date of Birth, Nationality, I.D. or passport number and date of issuance, Registry place and number in Lebanon
  • Name and Signature of two witnesses with their ID/passport number and date of birth ( must not be relatives of the applicant or recipient).
  • Notarized by the Notary Public. Must include the following statement on the same page signed by the applicant and the two witnesses:
    "Before me personally appeared Mr./Ms./Mrs. .....
    holding ID/passport number......
    and witnesses Mr./Ms./Mrs. ......
    holding ID/passport number......
    and signed the above document, this day .../.../... "
    then signature and seal of the Notary Public, and the expiration date of commission.
  • Authenticated by the Secretary of State.
  • Power of Attorney and a photocopy of it.
  • Copies of I.D.'s of witnesses, applicant and recipient (For Lebanese, a Lebanese ID is required).
  • Applicant must provide a clear address and correct contact number for the applicant.
  • Applicants over 73 years of age are kindly requested to submit, along with the required documents, a certificate of good mental health to be issued by their physician.
  • For general Power of Attorney a Bank Issued Cashier's Check or USPS Money Order Only in the amount of $100.00 per signature, per applicant payable to the Consulate General of Lebanon.
  • Stamped, self-addressed envelope (USPS ONLY).
  • All Power of Attorney received by mail (Special and General) composed of more than one page MUST include the signature of the applicant and the seal and sign of Notary Public on each and every page of Power of Attorney.
  • All Power of Attorney should include the full identity of both applicant and recipient: First and Last name, Father's name, Mother's name, Date of Birth, Nationality, I.D. or passport number and date of issuance, Registry place and number in Lebanon

  • Regarding the sale of property, the applicant must include a copy of the deed of trust. The number and location of the property also needs to be mentioned clearly in the power of attorney.
