General Information

• Download and fill out application
• Passport (must be valid for atleast 6 months)
• Please send by post with pre-paid self addressed envelope (The envelope send should be of a big size) the following:
  • Passport (Valid for at least 6 months)
  • A copy of the first page of your U.S. passport
  • A letter of employment
  • One (1) passport photo (2" x 2").
  • For minors: A signed and notarized letter from both parents stating their consent in issuing a visa.
  • The fee of $88.00 USD (One Single Entry), $125.00 USD (Three Month Entry), $175.00 USD (Multiple Entry).

    Only Cashiers checks or USPS Money Orders payable to the Consulate General of Lebanon in Detroit are accepted.
